Sick Note

When do I need a sick note?

7 days off sick or less

If you're off work sick for 7 days or less, your employer should not ask for medical evidence that you've been ill. Instead, they can ask you to confirm that you've been ill. You can do this by filling in a form yourself when you return to work. This is called self-certification.

Croft Hall also offers paper copies of these forms at reception, please ask a member of the admin team for assistance.

More than 7 days off sick

If you're off work sick for more than 7 days, your employer will usually ask for a sick/fit note (or Statement of Fitness for Work) from a GP or hospital doctor. Sick/fit notes are sometimes referred to as medical statements or a doctor's note.

Requesting a Sick Note

If you require a sick note if you have been off work for 7 days or more and have already self-certified for the first 7 days, please complete the below form.

Please note that you may be contacted by a GP to discuss the request. If you require an extension to a sick note, please contact the surgery on the date this ends.

Please note, sick note can be backdated but NOT post-dated.

Please do not write any clinical queries on this form. Any clinical queries or request for appointments via this form will be discarded. To make an appointment either book online via the NHS app/Patient access or by calling reception 01803 298441.

If your request is approved you will receive a text to confirm it is ready to collect.
PLEASE NOTE THAT WE CANNOT USE A DATE IN THE FUTURE. Either today or a date in the past.

Please note: if you experience a fever (>37.8C) or a persistent cough (>4 hours) do not attend to collect your sick note. Instead self-isolate for 7 days & collect once this is over.
